About the Role:
The Weekend Day Hospice LPN plays a critical role in delivering compassionate, high-quality nursing care to patients receiving hospice services during weekend hours. This position focuses on managing and monitoring patients' physical and emotional needs, ensuring comfort and dignity in end-of-life care. The LPN collaborates closely with interdisciplinary teams, including physicians, social workers, and family members, to implement individualized care plans. By providing skilled nursing interventions and patient education, the LPN helps to alleviate symptoms and improve quality of life for patients and their families. Ultimately, this role supports the mission of hospice care by fostering a supportive environment that respects patient wishes and promotes holistic well-being during challenging times.

The Weekend Day Hospice LPN utilizes clinical nursing skills daily to assess patient conditions, administer medications, and provide symptom relief, ensuring patient comfort and safety. Additionally, proficiency with electronic health records supports seamless information sharing and compliance with healthcare regulations.
